With your consent, we will also ask you to provide us with certain personal health information, such as the name and location of your doctor, health documentation, your prescribed daily amount of medicinal cannabis, the period of authorized use and the quantity of medicinal cannabis you wish to purchase. We may also ask you to provide your emergency contact information.

You may be directed to Pathway Health by your health care provider, such as your family physician, pharmacist or other health care professional. With your consent, your health care provider may need to share your personal health information with Pathway Health, so that we can provide our services to you.

  • Licensed Producers – to facilitate your registration so your medical cannabis order can be fulfilled. In addition, product ordering information is exchanged with Pathway Health, so that we can verify that the correct product was ordered, update healthcare providers in the circle of care and analyze medical cannabis usage trends to help improve our services.
  • Health Care Practitioners – In order to provide comprehensive care, it may be necessary for Pathway Health to share your personal health information with a Health Care Practitioner who is participating in your care, in connection with your use of medicinal cannabis.

Public Profiles. If you provide personal information to the Pathway Health Blog, this information will be publicly available and therefore may be seen by other users.

Trusted Third Party Service Providers. Pathway Health may use third parties to provide technical support and perform tracking and reporting functions, conduct quality assurance testing, and provide other services in connection with its services. Pathway Health may share your personal information with them so that they can perform the required services.

Third Parties. Upon receipt of a written request by a client or patient to share personal information with a third party, Pathway Health will provide the requested data.
